ABSTRACT

Objective:To explore a simple and effective method to improve the facial shape coordination for patients.Methods:From May 2015 to May 2020, 36 female patients (aged 24-32 years, mean 26.8 years) who were required to change face shape were enrolled in Zhengzhou Mylike Medical Cosmetology Hospital to improve face shape by adjusting frontaI arch width combined with autologous high-density fat transplantation.Results:Thirty-three patients (91.7%) were satisfied (31 satisfied, 2 basically satisfied). Three patients were not satisfied with the filling effect, and two of them were satisfied after the second filling (3 months after operation). 35 of the 36 patients were satisfied, accounting for 97.2%, only 1 case was dissatisfied. No other compIications such as fat liquefaction were observed. The mean morphological surface index of patients was 90.2 before operation and 88.1 after operation, showing significant improvement.Conclusions:Autologous high density fat transplantation is a good method to adjust the coordination of the frontal arch.

Introduction: Due to the extensive number of studies developed on periodontal pathologies and the clinical need generated to correct bonvze defects, we have carried out an Overview of systematic reviews using the FRISBEE methodology. Material and Methods: Through this study we expect to bridge the knowledge gap generated regarding the clinical question on the effectiveness of autologous bone substitutes and xenografts in maxillary and mandibular bone defects. Results: For this study, we carried out a systematic search in Epistemonikos and PubMed, we included 3 systematic reviews and 5 primary studies included in these reviews to extract their data. We analyzed data using RevMan 5.4. and GRADEpro. Assessed outcomes included: bone gain [MD 0.06 mm lower (0.26 lower to 0.14 higher)] and bone resorption [MD 0.03 mm higher (0.12 lower to 0.18 higher)], where no significant differences were found between the study groups. The certainty of the evidence was moderate for both outcomes. Bone length and bone density outcomes were not measured or reported in the included studies. Conclusion: We concluded that there are no significant clinical differences between the application of autologous bone grafts and xenografts for bone defects correction for the assessed outcomes, therefore, these biomaterials should be applied at the discretion of the clinician and according to the needs and preferences of patients.


Introducción: Debido al extenso número de estudios desarrollados sobre patologías periodontales y a la necesidad clínica generada para corregir defectos óseos, hemos realizado un Overview de revisiones sistemáticas tipo FRISBEE para acortar la brecha de conocimiento generada respecto a la pregunta clínica sobre la efectividad de sustitutos óseos tipo autólogo y xenoinjertos en defectos óseos a nivel maxilar y mandibular. Material y Métodos: Para este estudio realizamos una búsqueda sistemática en Epistemonikos y PubMed, de los cuales incluimos 3 revisiones sistemáticas y 5 estudios primarios incluidos en estas revisiones para extraer sus datos. Los datos fueron analizados a través de RevMan 5.4. Y GRADEpro. Resultados: Los estudios analizaron los desenlaces propuestos: ganancia ósea posterior a la aplicación del injerto óseo [MD 0.06 mm menos (0.26 menos a 0.14 más)] y reabsorción ósea posterior a la aplicación del injerto óseo [MD 0.03 mm más (0.12 menos a 0.18 más)], donde no se encontraron diferencias significativas entre los grupos de estudio. La certeza de la evidencia fue moderada para ambos desenlaces. Los desenlaces longitud ósea y densidad ósea no fueron medidos o reportados en los estudios incluidos. Conclusión: Se concluyó que no hay diferencias que sean clínicamente significativas entre la aplicación de injertos óseos autólogos y xenoinjertos para la corrección de defectos óseos para los desenlaces analizados, por lo que, la aplicación de estos biomateriales queda a criterio del clínico, y de acuerdo a las necesidades y preferencias de los pacientes.

Introdução: A remoção de implantes mamários de silicone é uma cirurgia com crescente notoriedade. O avanço nos estudos sobre o linfoma BIA-ALCL (Breast Implant Associated Anaplastic Large Cell Lymphoma) e patologias inflamatórias/autoimunes relacionadas aos implantes, como a síndrome de ASIA (Autoimmune Syndrome Induced by Adjuvants), tem gerado desconforto em muitas pacientes, que optam pela sua retirada. Essa demanda traz ao cirurgião um cenário muitas vezes desafiador, no qual é preciso devolver às mamas um formato anatômico e esteticamente agradável após a retirada desse material. O objetivo é demonstrar estratégias cirúrgicas para o explante mamário de silicone. Métodos: Foram avaliadas 20 pacientes submetidas ao explante mamário, entre setembro de 2020 e março de 2021, incluindo explantes simples e associados à mastopexia. A indicação cirúrgica foi a demanda explícita das pacientes, motivadas por queixas como receio de patologias associadas, descontentamento estético, desconforto nas mamas, sintomas sistêmicos, desejo de não ter mais implantes, contratura capsular, rotação ou ruptura dos mesmos. As técnicas cirúrgicas são descritas. Resultados: Foi realizado o explante simples em 7 pacientes e em 13 foi feito o explante com mastopexia. Em 17 pacientes foi associada a lipoenxertia. A capsulectomia foi executada em todos os casos. Nas mastopexias, usamos o retalho dermoglandular de pedículo inferior para preservação de tecido mamário, associando também a lipoenxertia. Conclusão: As técnicas descritas conseguem devolver às mamas formato satisfatório, com baixas taxas de complicação e bom nível de aceitação das pacientes. Apesar das patologias relacionadas aos implantes de silicone até o momento mostrarem-se estatisticamente raras, é fundamental que estejamos atentos a sintomas e capacitados para oferecer opções cirúrgicas para o explante.


Introduction: Breast implants explantation surgery has been increasing in notoriety. Advances in studies on lymphomas BIA-ALCL (Breast Implant Associated Anaplastic Large Cell Lymphoma) and inflammatory/autoimmune pathologies related to the implants, like ASIA (Autoimmune Syndrome Induced by Adjuvants), has generated discomfort in many patients that choose to remove it. This type of demand, where it is necessary to return the breasts to an anatomical and aesthetically pleasing shape after the removal of the implants, might be a challenge for surgeons. The objective is to demonstrate surgical strategies for breast silicone explantation. Methods: Twenty patients who underwent the breast explantation surgery between September 2020 and March 2021 were evaluated. The surgical indication was always the patients demand, motivated by fear of associated pathologies, aesthetic dissatisfaction, discomfort in the breasts, systemic symptoms, desire to have no more breast implant, capsular contracture, rotation or rupture of the implant. Surgical techniques are described. Results: The simple explant was performed in 7 patients, and in 13 the explant was performed with mastopexy. In 17 patients, fat grafting was associated. Capsulectomy was performed in all cases. In mastopexies, we used the inferior dermoglandular flap to preserve breast tissue, also associating fat grafting. Conclusion: Techniques described are able to return the breasts to a satisfactory shape, with low complication rates and good levels of patient acceptance. Despite the pathologies related to silicone implants so far show themselves statistically rare, it is essential that we are attentive to symptoms and trained to provide surgical strategy for the explant.

ABSTRACT Objective: To describe results of pterygium surgery at Clínica de Cirurgía Ocular Villavicencio, in Meta, Colombia. Methods: A retrospective analysis of pterygium surgeries performed between January 2017 and December 2019. Results: Approximately 1,200 records were reviewed, and 1,200 procedures included. The most frequent complications were corneal Dellen, pterygium recurrence, graft retraction and pyogenic granuloma (7.5%, 2.5%, 3% and 0.75% respectively). These were identified within the first 6 months of surgery. Conclusion: Pterygium surgery using conjunctival autografts is an effective technique, with low recurrence rates and very few complications.


RESUMO Objetivo: Descrever os resultados da cirurgia de pterígio na Clínica de Cirurgía Ocular Villavicencio, Meta, Colômbia. Métodos: Análise retrospectiva de cirurgias de pterígio realizadas entre janeiro de 2017 e dezembro de 2019. Resultados: Aproximadamente 1.200 prontuários foram revisados e 1.200 procedimentos foram incluídos. As complicações mais frequentes foram o Dellen corneano, a recorrência do pterígio, a retração do enxerto e o piogranuloma (7,5%, 2,5%, 3% e 0,75%, respectivamente). Essas complicações foram identificadas nos primeiros 6 meses após a cirurgia. Conclusão: A cirurgia de pterígio pela técnica de autoenxerto conjuntival é um procedimento eficaz, com baixos índices de recorrência e poucas complicações.

■ RESUMO Introdução: A pandemia da COVID-19 foi responsável pelo aumento dos índices de isolamento social, resultando na diminuição de acidentes de trânsito. Traumas em geral estão entre os problemas de saúde pública mais importantes em todo o mundo. O objetivo deste trabalho é avaliar o impacto do isolamento social na cidade de São Paulo durante a pandemia da COVID-19 nos atendimentos de pacientes com ferimentos descolantes. Métodos: Estudo observacional, retrospectivo e descritivo. Foram revisados todos os prontuários de pacientes atendidos no Pronto Socorro do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP), pelo Grupo de Feridas Complexas do Serviço de Cirurgia Plástica, admitidos por ferimentos descolantes e submetidos a procedimento cirúrgico, no período de abril a junho dos anos de 2019 e 2020. Resultados: Foram incluídos 20 pacientes com ferimentos descolantes em membros inferiores. Em 2019, foram atendidos 14 pacientes, com idade média de 47 anos, sendo 7 vítimas de acidente de trânsito, 5 de atropelamento, 1 de esmagamento e 1 de queda da própria altura. Em 2020, durante a pandemia, foram atendidos 6 pacientes com ferimento descolante, com idade média de 36,16 anos, sendo que 5 foram vítimas de acidentes com motocicletas e 1 de atropelamento. Nenhum paciente apresentou COVID-19. Conclusão: Durante o isolamento social houve redução numérica nos atendimentos de traumas descolantes no período de quarentena.


■ ABSTRACT Introduction: The COVID-19 pandemic was responsible for increasing social isolation rates, resulting in a reduction in traffic accidents. Trauma, in general, is among the most important public health problems worldwide. This study aims to evaluate the impact of social isolation in the city of São Paulo during the COVID-19 pandemic on the care of patients with detaching wounds. Methods: Observational, retrospective and descriptive study. All medical records of patients treated at the Emergency Room of the Hospital das Clínicas of the Faculty of Medicine of the University of São Paulo (HCFMUSP), by the Complex Wounds Group of the Plastic Surgery Service, admitted for detaching wounds and undergoing surgical procedures, were reviewed, from April to June of the years 2019 and 2020. Results: Twenty patients with detaching injuries on the lower limbs were included. In 2019, 14 patients were seen, with a mean age of 47 years, 7 of which were victims of traffic accidents, 5 of being run over, 1 of crushing and 1 of falling from standing height. In 2020, 6 patients were treated with detaching wounds during the pandemic, with a mean age of 36.16 years, 5 of which were victims of motorcycle accidents and 1 of being run over. No patient had COVID-19. Conclusion: During social isolation, there was a numerical reduction in care for detaching trauma during the quarantine period.

Objectives: To evaluate the use of extracted autogenous teeth for socket preservation after tooth extraction. Material and Methods: Cochrane, Scopus, and PubMed databases search was conducted to identify human clinical studies reporting the clinical, radiographic and/or histological outcomes of socket preservation techniques with autogenous extracted tooth Only studies published in English language in the last 10 years were included in the study. Results: In total, 82 articles were identified. Five articles were included in the review. They included 58 teeth that were prepared as a graft for socket preservation. The grafts derived from autogenous teeth were presented in three forms: particles, blocks and powder. The mean bone loss ranged from 0.28 mm to 0.41mm in height and 0.15 mm in width. Conclusion: Immediate autogenous extracted tooth as a grafting material for fresh socket preservation is promising for future daily clinical practice. More clinical comparative studies are needed.


Objetivo: Evaluar el uso de dientes extraídos autógenos para la preservación del alveolo tras la extracción dental.Material y Métodos: Se realizó una búsqueda en las bases de datos Cochrane, Scopus y PubMed para identificar estudios clínicos en humanos que informaban los resultados clínicos, radiográficos y / o histológicos de las técnicas de preservación de alveolos con dientes extraídos autógenos. Solo se incluyeron estudios publicados en inglés en los últimos 10 años. Resultados: En total se identificaron 82 artículos. Se incluyeron cinco artículos en la revisión. Incluyeron 58 dientes que se prepararon como injerto para la preservación del alveolo. Los injertos derivados de dientes autógenos se presentaron en tres formas: partículas, bloques y polvo. La pérdida ósea media osciló entre 0,28 mm y 0,41 mm de altura y 0,15 mm de ancho. Conclusión: El diente autógeno extraído de forma inmediata utilizado como material de injerto para la conservación del alveolo fresco es prometedor para la práctica clínica diaria futura. Se necesitan más estudios clínicos comparativos.

Objective:To investigate the clinical significance of suction blister transplantation in improving the efficacy of ReCell technique in the treatment of vitiligo.Methods:Patients were divided into three groups, namely, vitiligo patients without history of suction blister therapy, patients with ineffective suction blister therapy and patients with effective suction blister therapy. There were 30 patients in each group. All patients were treated with standard procedure of ReCell technique. The color recovery effect of leukoplakia was observed 3 and 6 months after operation, and the incidence of complications was also observed.Results:The effective rate of color recovery 3 and 6 months after operation were as follow: in patients without history of suction blister group, the effective rate of three months was 53.3%, and that of six months was 63.3%; in patients with ineffective suction blister group, the effective rate was 43.3% in three months and 50.0% in six months, and in patients with effective suction blister group, the effective rate was 76.7% in three months and 90.0% in six months. No obvious complications were observed in the three groups.Conclusions:For the treatment of stable vitiligo with ReCell technique, suction blister method is a simple and effective method for screening patients.

Introdução: A pressão positiva contínua nas vias aéreas nasais (CPAP) é uma forma não invasiva de ventilação utilizada em recém-nascidos prematuros em unidades de terapia intensiva. Porém, ela pode acometer o nariz destes pacientes, evoluindo até mesmo com isquemia e necrose columelar. Diversas técnicas são descritas para reconstruir a columela, como enxertos de pele, enxertos compostos, retalhos locais e retalhos livres; mas o enxerto composto condrocutâneo auricular tem se destacado. O objetivo deste trabalho é descrever um caso de necrose de columela por uso de CPAP com reconstrução utilizando enxertia condrocutânea auricular posterior. Relato do Caso: Paciente do sexo feminino, parda, com história de prematuridade e uso prolongado de CPAP quando recém-nascida devido à síndrome da membrana hialina, evoluiu com necrose de columela. Foi submetida à enxertia composta condrocutânea auricular posterior para reconstrução da columela. A paciente apresentou resultado cirúrgico satisfatório, evoluindo com 100% de vitalidade do enxerto. Discussão: A necrose columelar associada ao uso do CPAP pode ser estética e funcionalmente debilitante, e representa um desafio reconstrutivo. As opções para obter resultados aceitáveis são limitadas, porém o uso de enxertos de orelha é tecnicamente simples, utiliza tecidos doadores estruturalmente semelhantes, não provoca cicatrizes adicionais no nariz, é realizado em um tempo cirúrgico e geralmente tem um resultado muito bom. A enxertia composta auricular posterior para reconstrução columelar se mostrou segura, com resultado estético e funcional satisfatório e mínima morbidade da área doadora.


Introduction: Continuous positive pressure in the nasal airways (CPAP) is a non-invasive form of ventilation used in premature newborns in intensive care units. However, it can affect the nose of these patients, even evolving with ischemia and columellar necrosis. Several techniques are described to reconstruct the columella, such as skin grafts, composite grafts, local flaps, and free flaps, but the atrial chondrocutaneous graft has stood out. This study aims to describe a case of columella necrosis using CPAP with reconstruction using posterior atrial chondrocutaneous grafting. Case Report: A brown, female patient, with a history of prematurity and prolonged use of CPAP when she was born due to hyaline membrane syndrome, developed columella necrosis. The patient underwent posterior auricular chondrocutaneous grafting to reconstruct the columella. She presented a satisfactory surgical result, evolving with 100% graft vitality. Discussion: Columellar necrosis associated with the use of CPAP can be aesthetically and functionally debilitating, and represents a reconstructive challenge. The options for obtaining acceptable results are limited. However, the use of ear grafts is technically straightforward, uses structurally similar donor tissues, does not cause additional scarring on the nose, is performed in a surgical period, and generally has an excellent result. Posterior auricular composite grafting for columellar reconstruction proved safe, with satisfactory aesthetic and functional results and minimal morbidity in the donor area.

@#AIM: To compare the effect of platelet-rich fibrin(PRF)grafts and conjunctival-limbal autografts in pterygium excision.<p>METHODS: Totally 62 cases(62 eyes)of primary pterygium were randomly divided into group 1(32 eyes)and group 2(30 eyes). After pterygium excision, conjunctival-limbal autografts was performed in group 1 and PRF grafts was performed in group 2. PRF was prepared by centrifugation and compression of venous blood from patients before the surgery. The follow-up period was 6mo. The surgery time, complications and recurrence rate were evaluated and compared between the two groups.<p>RESULTS: The operation time of group 1 and group 2 were 27.3±4.3min and 22.0±4.0min respectively. There was significant difference between the two groups(<i>t</i>=4.990, <i>P</i><0.01). Conjunctival granuloma developed only in one eye in group 2 after the surgery, and there was no significant difference between group 1 and group 2(<i>P</i>=0.484). Pterygium recurred in one eye in group 1 and in one eye in group 2. There was also no significant difference in recurrence rate between group 1 and group 2(<i>P</i>=1.000). There was no significant difference in intraocular pressure between group 1 with 14.69±2.44mmHg and group 2 with 14.96±2.93mmHg before the surgery(<i>t</i>=0.399, <i>P</i>=0.691). As well, there was no significant difference in intraocular pressure between group 1 with 14.68±1.65mmHg and group 2 with 15.11±2.12mmHg after the surgery(<i>t</i>=0.888, <i>P</i>=0.378). The dissolution time of PRF membrane in PRF grafts group was 3.5±0.8d.<p>CONCLUSION: The use of PRF in pterygium surgery is safe, effective, easier and timesaving. It's a promising method for clinical application.

ABSTRACT

ABSTRACT Multiple sclerosis (MS) is an immune-mediated disease of the central nervous system. Its treatment has focused on inflammation control as early as possible to avoid disability. Autologous hematopoietic stem cell transplantation (AHSCT) has been used for treating MS since 1996, with recent decisive results regarding benefits in long-term efficacy. Five patients followed up at an MS center in Belo Horizonte, Brazil, who had relapsing-remitting MS with high disease activity, underwent AHSCT between 2009 and 2011. They were evaluated clinically, with magnetic resonance imaging, and by the EDSS every six months after transplantation, up to July 2018. The patients were four women and one man, with ages ranging from 25-50 years, and time since disease onset ranging from 4-17 years at the time of the procedure. Four patients improved, one patient was stabilized, and all patients were free of disease activity after 5-9 years. Through improving patient selection and decreasing the time from disease onset, AHSCT could stop epitope spreading and disease progression. Despite multiple other therapeutic choices being approved for relapsing-remitting MS, AHSCT continues to be a treatment to consider for aggressive MS disease.


RESUMO A esclerose múltipla é uma doença imunomediada do sistema nervoso central. Seu tratamento tem sido focado no controle da inflamação o mais cedo possível para evitar incapacidade. O transplante autólogo de células tronco hematopoiéticas (TACTH) vem sendo usado para tratar esclerose múltipla desde 1996, e recentes resultados foram decisivos a respeito do benefício na eficácia a longo prazo. Cinco pacientes seguidos num centro de esclerose múltipla de Belo Horizonte, Brasil, que apresentavam forma clínica remitente recorrente com alta atividade de doença foram submetidos a esse tratamento de 2009 a 2011. Após o transplante foram avaliados clinicamente e com ressonância magnética, e escala de EDSS, a cada seis meses até julho de 2018. Os pacientes eram quatro mulheres e um homem, com idade entre 25 e 50 anos e tempo de doença variando de 4 a 17 anos na época do procedimento. Quatro pacientes melhoraram após a primeira avaliação e um manteve-se estável. Todos os pacientes permaneceram sem evidência de doença clínica ou radiológica de 5 a 9 anos após. Com uma seleção cada vez mais criteriosa de pacientes o TACTH pode interromper a disseminação do epítopo e controlar a progressão da doença. Apesar de várias outras opções terapêuticas aprovadas para esclerose múltipla o TACTH continua a ser uma forma de tratamento a ser considerada em casos de doença muito agressiva.

Objective@#To investigate the clinical application of injected autologous fat and buried polydioxanone threads for nasolabial fold filling.@*Methods@#From April 2016 to October 2017, 64 cases of mild and moderate nasolabial fold beauty seekers from the First People's Hospital of Jiande were divided into group A and group B with their attitude. Group A (28 cases) were only treated with buried polydioxanone threads for nasolabial fold filling; Group B (36 cases) were treated with injected autologous fat and buried polydioxanone threads for nasolabial fold filling. The surgical effect was recorded for 18 months.@*Results@#The most common complications were swelling, bruising and pain immediately after the surgery. Polydioxanone threads exposure occurred in two cases in each group 2 weeks after the surgery. There were satisfactory results in skin color and skin texture in both groups. 9 cases showed inadequate correction in group A while only 2 cases showed inadequate correction in group B in long-term follow-up (>3 months).@*Conclusions@#Combined treatment of autologous fat and polydioxanone threads is safe and effective for nasolabial fold filling.

Objective@#To discuss the use of butterfly costal cartilage as columella strut graft.@*Methods@#On the costal cartilage that being cut, butterfly graft was designed length of 2.8 cm, upper and lower end thickness of 5 mm, middle part thickness of 1.5 mm. The lower end was designed with a groove about 1 cm in length, the width of the lower end of the butterfly graft was determined according to the degree of retraction at the caudal end of the septum, then insert to the nasal crest. The posterior end of two lamella grafts was sutured and fixed with the middle part of the nasal columnar strut graft.@*Results@#From November 2016 to March 2018, about 150 patients underwent rhinoplasty with this method, follow-up was 3 to 12 months. Two cases had mild nasal tip deviation one month after surgery and were adjusted with auricular cartilage three months later. Three patients had mild postoperative hyporotation and adjustment three months postoperatively, residual had good shape.@*Conclusions@#The stability and controllability of the sphenoid columella strut graft are good, out of shape not easily, It is a worthy clinical method.

ABSTRACT

Objective To conclude the experience of restoration of fat compartments volume intreatment of facial volume lost aging.Methods From May 2015 to May 2018,126 patients with facialvolume lost aging,aged from 34 to 52 years (mean age 42.6 years),were corrected by restoring fatcompartments volume using different sized autologous fat granule grafting technique.Patients' satis-faction was rated using Visual Analogue Scale (VAS) ranging from 1 (most unsatisfactory) to 10(most satisfactory).Results 126 patients with volume lost aging face were followed up from 3months to 2.5 years,the sculpted appearance became more fullness,the laxity,wrinkles and textureof skin were improved obviously,the patients' VAS score was (8.2±1.4).Conclusions Restorationof fat compartments by using different sized autologous fat granule grafting can augment the facicalfullness,reshape the facial contour and improve the aging skin.Therefore it proves to be a safe,multi-effecive and satisfying technique in the treatment of facial volume lost aging.

ABSTRACT

PURPOSE: Bone flap resorption (BFR) after cranioplasty with an autologous bone flap (ABF) is well known. However, the prevalences and degrees of BFR remain unclear. This study aimed to evaluate changes in ABFs following cranioplasty and to investigate factors related with BFR. MATERIALS AND METHODS: We retrospectively reviewed 97 patients who underwent cranioplasty with frozen ABF between January 2007 and December 2016. Brain CT images of these patients were reconstructed to form three-dimensional (3D) images, and 3D images of ABF were separated using medical image editing software. ABF volumes on images were measured using 3D image editing software and were compared between images in the immediate postoperative period and at postoperative 12 months. Risk factors related with BFR were also analyzed. RESULTS: The volumes of bone flaps calculated from CT images immediately after cranioplasty ranged from 55.3 cm³ to 175 cm³. Remnant bone flap volumes at postoperative 12 months ranged from 14.2% to 102.5% of the original volume. Seventy-five patients (77.3%) had a BFR rate exceeding 10% at 12 months after cranioplasty, and 26 patients (26.8%) presented severe BFR over 40%. Ten patients (10.3%) underwent repeated cranioplasty due to severe BFR. The use of a 5-mm burr for central tack-up sutures was significantly associated with BFR (p<0.001). CONCLUSION: Most ABFs after cranioplasty are absorbed. Thus, when using frozen ABF, patients should be adequately informed. To prevent BFR, making holes must be kept to a minimum during ABF grafting.

ABSTRACT

Objective To observe the clinical effect of water-assisted liposuction system using in the breast augmentation surgery.Methods From 2015 to 2017,we received 30 new patients who were diagnosed as small breast disease.The water-assisted liposuction system (Body-jetTM) was used to harvest autologous fat,and then autologous fat grafting was preformed in the breast augmentation surgery.The follow-up was conducted one year after operation to observe the change of breast size,fat survival rate,and the occurrence of postoperative complications.Results There was no any serious complication happened in all the 30 cases.The breasts got a good shape and soft feeling after long-term follow up.Conclusions The application of body-jetTM water-assisted liposuction system is safe;the survival rate of autologous fat transplantation is high,and the clinical effect is satisfactory.

PURPOSE@#To compare the efficacy and safety of recombinant human bone morphogenetic protein (rhBMP) and iliac crest autograft in the fusion treatment of lumbar spondylolisthesis.@*METHODS@#The studies using randomized controlled trials to compare the rhBMP with iliac crest autograft in the treatment of lumbar spondylolisthesis were retrieved from Embase, Pubmed, ProQuest dissertations & theses (PQDT), China national knowledge infrastructure (CNKI), Chinese Biomedical Database, Wanfang Data, Cochrane Library (from March 1998 to March 2018). Postoperative fusion rate, clinical success rate, postoperative intervertebral height, complications, operation time, blood loss and duration of hospitalization were chosen as the outcome indicators. Methodological quality of the trials was critically assessed, and relevant data were extracted. Statistical software Revman 5.3 was used for data-analysis.@*RESULTS@#Eleven articles were included in the meta-analysis. The results showed that, comparing the efficacy of rhBMP with iliac crest autograft, statistical significance was found in the 24-month fusion rate post operation [95% CI (1.38, 24.70), p = 0.02] and operation time [95% CI (-14.22, -2.08), p = 0.008]. There is not sufficient evidence for statistical differences in the remaining indicators.@*CONCLUSION@#The current literature shows rhBMP is a safe and effective grafting material in the treatment of lumbar spondylolisthesis. Further evidence is dependent on the emergence of more randomized controlled trials with higher quality and larger sample sizes in the future.

ABSTRACT

BACKGROUND: Trauma is one of the most common causes of enophthalmos, and post-traumatic enophthalmos primarily results from an increased volume of the bony orbit. We achieved good long-term results by simultaneously using an anatomical absorbable implant and iliac bone graft to correct post-traumatic enophthalmos.METHODS: From January 2012 to December 2016, we performed operations on seven patients with post-traumatic enophthalmos. In all seven cases, reduction surgery for the initial trauma was performed at our hospital. Hertel exophthalmometry, clinical photography, three-dimensional computed tomography (3D-CT), and orbital volume measurements using software to calculate the specific volume captured on 3D-CT (ITK-SNAP, Insight Toolkit-SNAP) were performed preoperatively and postoperatively.RESULTS: Patients were evaluated based on exophthalmometry, clinical photographs, 3D-CT, and orbital volume measured by the ITK-SNAP program at 5 days and 1 year postoperatively, and all factors improved significantly compared with the preoperative baseline. Complications such as hematoma or extraocular muscle limitation were absent, and the corrected orbital volume was well maintained at the 1-year follow-up visit.CONCLUSION: We present a method to correct enophthalmos by reconstructing the orbital wall using an anatomical absorbable implant and a simultaneous autologous iliac bone graft. All cases showed satisfactory results for enophthalmos correction. We suggest this method as a good option for the correction of post-traumatic enophthalmos.

BACKGROUND: Various methods have been reported for the improvement of widened or depressed scars. However, scars can be complex at presentation. If a widened but flat portion is combined with a tethered or a depressed area, a scar revision method that can effectively resolve all issues is needed. The authors utilized a dermal portion of the widened scar as a re-adhesion barrier and filled tissue after the release of the tethering or depression. METHODS: From July 2014 to December 2017, a total of eight patients presented with combined scars with both widened and depressed areas and underwent scar revisions with scar dermal transposition flaps. The scar flap of the widened scar was de-epithelialized, leaving the dermo-cutaneous pedicle near the depressed area. Without any additional skin incision on the depressed site, the tethering was released by making a skin incision at the scar flap. The de-epithelialized scar dermal flap was transpositioned under the depressed area of the scar. RESULTS: The surgical wounds of all eight patients healed without any complications. The mean follow-up period was 5.25 months. The filling effect of the scar flap persisted without the conspicuous relapse of a depression or tethering. The patients were satisfied with the final results and the fact that no additional incision was needed for the tethered and depressed scar. CONCLUSIONS: If the depressed site is near a widened scar, a scar dermal transposition flap may be a versatile option for improving the depression without an additional skin incision.

ABSTRACT

Objective To observe the clinical effects of external tissue expansion assisted lipotransfer for temple depression augmentation.Methods A total of 18 cases of temple depression wore the a vacuum-based cupping cup device for recipient-site preconditioning for 1-3 months before fat injection.Results Temple soft tissue can pinched easily after expansion,fat injection ranged from 12-16ml per site,the temple depression were augmented and the sculpted appearance were improved obviously after 1to 12 months follow-up,there were no complications such as nodules,masses,alopecia or cysts occurred.Patient satisfaction was assessed and the VAS score was (7.8.± 1.4).Conclusions The addition of temple region expansion before fat injection can improve the skin compliance,increase the recipient-site volume,enhance fat graft placement and higher graft survival rates,demonstrating a high safety and efficacy procedure.

ABSTRACT

Objective To measure the change of facial volume maintenance rate after autologous fat grafted for repaired progressive facial hemiatrophy by using three-dimensional digital technology.Methods 3D scanner was used to acquire facial data in 10 patients with progressive facial hemiatrophy before operation;Mimics 17.0 software was used to reconstruce patients' facial 3D model and to calculate the volume of facial tissue defect;autologous fat was grafted to repair facial deformity.The facial volume maintenance rate was calculated in all the patients 3 months and 6 months after operation.Results We had performed facial 3D data acquisition and facial repaired with autologus fat grafted in 10 patients;patients' facial morphology was improved.The mean facial volume maintenance rate was (35.80±3.44)% in 3 months and (27.82±3.80)% 6 months after surgery.Conclusions The mean facial volume maintenance rate in postoperative 3 months is inferior to that in 6 months in single autologous fat grafted for repairing progressive facial hemiatrophy.
